How will care be arranged?
A Dependability manager will visit you and your family at home or in hospital to agree what assistance you require.
We will listen and take account of your needs. Based on our visit we will produce a Care Plan. Each visit will be carefully tailored to your particular needs, and the Care Plan will be regularly reviewed with you.
Deciding your care needs
Many people requiring care for the first time don’t know what kind of support they require.
To help you decide the areas you need assistance with, we will carry out an assessment to evaluate your care needs. and discuss this with your and/or your family to ensure we understand how to fully support you and provide advice.
Terminology
‘Home Care Services’ and ‘Personal Care’ are general terms often used when people need extra help with household cleaning, preparing meals, getting dressed, or support with shopping.
A 'Care Package' is a combination of services put together to meet a person’s need as part of a Care Plan.
A 'Care Plan' defines exactly what a person needs to properly be cared for, and forms the basis of the care contract.
